Активный участник боевых действий на Донбассе до начала СВО. Подразделение: 15-й отдельный горно-пехотный батальон. Дата и место гибели: 31 января 2015 года, посёлок Редкодуб, Шахтёрский район, ДНР. Обстоятельства гибели: В ночь на 31 января группа карателей, в которой был Прошак, выдвинулась с опорного пункта на БМП-2, и была уничтожена по дороге в г. Дебальцево.
